Funeral Homes:
To use the Online Service Portal to order Death Certificates, please create a profile (Username and Password) by using the link here: New Portal Profile Registration.
- Please note that if you do not receive an automated verification email immediately after submitting your new profile registration, then this typically indicates that your organization's firewall or spam filter has blocked it. You will need to contact your email account administrator or IT person to request that emails from our system be allowed through. The email address to have them allow is noreply@clark.wa.gov
After you have successfully logged in to the portal, you will need to submit a new online funeral home account for each funeral home within your organization by using the link here: Death Certificate-Funeral Home-Registration Note:
- Please only submit one account registration form per funeral home, additional portal users can create their own profile in the portal but must be added by our staff after the new account registration form is processed. Within one business day, you will be notified via email by our staff that you may log back in to the portal and begin to place orders on your new account. Instructions on using the portal will be included. Note: You may need to log out of the portal and then log back in again to see the changes.
